The Severity of Dependence Scale (SDS)

The Severity of Dependence Scale (SDS) is a validated questionnaire that asks respondents a series of items about the severity of their dependence on opioids. The Severity of Dependence Scale was developed as a way of measuring the psychological aspects of dependence that those who are dependent on illicit substances experience. It asks questions about anxiety, preoccupation, and impairment related to the respondent’s drug use. Each of the five items is scored on a 4-point scale. The total score is obtained through the addition of the 5-item ratings. The higher the score the higher the level of dependence.
